1. RIGHTS AND AUTHORITY
Rights holder's correct legal name; representative's role.
Relevant China trademark numbers and current status, or the copyright work and ownership chain.
Assignments, licences and any known limits on enforcement authority.

2. ALLEGED ACTS
Full listing URLs, seller/shop IDs, platform, date and publicly available business information.
Original captures with context; do not alter the underlying records.

3. SAMPLES AND TRANSACTIONS
If a sample has been lawfully obtained: order, payment, communications, shipping label, parcel and packaging.
Record receipt/opening and keep the sample identifiable.
Authentic reference material and an explanation of the observed differences.

4. OBJECTIVE AND TIMING
Desired result: removal, cessation, compensation, source action or assessment of a suspected offence.
Actual known deadlines or urgent shipments, including the relevant time zone.
Prior notices, complaints, settlements or court/authority decisions.
